Concussion Vital Signs is free to all schools in the United States.

When registering for an account, please attempt to use an email address with an *.edu, *.us, *.org top level domain name (JSmith@schoolsystem.edu or Jsmith@jonescounty.us or JSmith@schoolsystem.org). Registering with a top level domain name that is not *.edu or *.us, *.org some other domain name typically used by schools, may delay or cancel your account. In some cases, ATCs may work outside the school. Should you work for a hospital or physician practice, you should register with the email associated with those practices.

Clubs or other entities that are not schools may be able to purchase an instance of Concussion Vital Signs. Email support@concussionvitalsigns.com, with any requests to purchase Concussion Vital Signs.

One-on-one training

The manuals and supporting materials on this website are very comprehensive and the overwhelming majority of users do not require "one-on-one" training. If it would be helpful to have an individual training session, the cost is $250 and is payable in advance. The request must originate from the email address of record and include your account number. To set up an individual webinar email support@concussionvitalsigns.com.

Telephone support

Telephone support is available for a fee. It must be scheduled in advance. Email support@concussionvitalsigns.com and provide three time and dates when you would like to speak. The request must originate from the email address of record and include your account number and the topic you would like to discuss. There is a charge of $65 per phone call(20 minutes or less). Have your account number and credit card number available at the start of the call. Your credit card will be charged at the beginning of the call.

Data for research projects

Universities and other schools wanting a tab delimited data set of their data for research purposes may request an export of the account data. Data requests must originate from the email account of record. The cost for a data download is $150 and is payable in advance.

Lost Administrative Login User Name

Your Administrative Login User Name is the email address provided as your user name at the time of registration.

If you serve a school, and are unable to remember your Administrative User Name and need to have it researched please send by courier (FedEx/UPS etc) a letter on school letterhead along with a Concussion Vital Signs report for any athlete, requesting we attempt to recover the Administrative Login, include in the letter your name, what you think the email address for the Administrative Login is, your phone number as well as the school principal/head master name, address and office phone number. We will reset the user name and provide the information to the principal/headmaster.

If you serve a college or university, and are unable to remember your Administrative User Name and need to have it researched please send by courier (FedEx/UPS etc) a letter on school letterhead along with a Concussion Vital Signs report for any athlete, requesting we attempt to recover the Administrative Login, include in the letter your name, what you think the email address for the Administrative Login is, your phone number as well as the athletic director’s name, address and office phone number. We will reset the user name and provide the information to the athletic director.

The cost the recover the Administrative Login User Name is $100.00 paid in advance by check from the school, college or university needing the Administrative Login. The payment should be included in the original letter asking for the attempted recovery.
